Once the order has been paid for, the customer's 'subscription_item_quantity' metafield is automatically increased by the number of additional slots that were purchased.

NB. This is only valid until the current billing period renews, at which point it will be reset to the default number of slots (eg. 5). It will also be reset once the customer's items are marked as with courier.
